What is Care Management?
The issues associated with aging are often complex and emotionally charged. At times like these, older adults and their families need someone with both expertise and empathy to help them consider options and make the best decisions for their unique situations. Home Care Assistance and our Care Managers are here to help.
In addition to top-quality, highly trained, compassionate, bonded and insured caregivers, each of our clients and their families also receive the services of a Care Manager. Home Care Assistance is the only company in our region to incorporate professional elder care management with premium home care service.
More than basic home care, from the beginning.
Our Care Managers are health and human services professionals. They hold degrees in nursing, social services, economics and other disciplines as well as a specialized body of knowledge and experience in aging and healthy longevity. Care Managers are trusted advisors to our clients and their families on a wide range of health, psychological, social, functional, legal and financial issues.
A Real Care Plan.
Care Managers perform a comprehensive assessment of our clients’ needs, develop a detailed Care Plan, help select the most appropriate caregivers and then personally train each caregiver on the client’s individualized plan and needs. A Care Manager then stays engaged in each client’s care with regular visits to review and modify the Care Plan as necessary, make certain the highest quality standards are upheld, introduce new caregivers to the client’s team, and provide consistent communication with our clients and their families.
A single point of contact.
A Care Manager is the single point of contact for our clients and their families and is ultimately responsible for ensuring they are completely satisfied with Home Care Assistance. This level of care management is included with our home care services at no additional charge.
How You Will Age is Uncertain—Our Care is Guaranteed.
Managing the health and wellness needs of older adults can be complex, confusing and fraught with logistical and emotional challenges. This is particularly true as life becomes busier and families may not be well positioned to provide the time required to assist their elderly loved ones. Home Care Assistance’s Care Managers ease the burden, and help our clients and their families achieve the independence, dignity, control and peace of mind they desire.
We are so confident you will be completely satisfied with our services that, if you are not after 30 days and decide to employ another home care agency with a lower cost, we will refund the difference in price.
When More Is Needed; Upon Request.
